This is a full, remote position with regular local travel (75%) required in TN<\/p>\r\n

Job Description:<\/p>\r\n

The Field Case Manager is responsible for assessment, planning, coordination, implementation and evaluation of injured\/disabled individuals involved in the medical case management process. Working as an intermediary between carriers, attorneys, medical care providers, employers and employees, you will closely monitor the progress of the injured worker and report results back to the employer and insurance carrier. This will ensure appropriate and cost-effective healthcare services leading to a medically rehabilitated individual who is ready to return to an optimal level of work and functioning. You will:
\u2022 Use clinical skills to help coordinate the individual\u2019s treatment program while maximizing cost containment.
\u2022 Serve as an intermediary to interpret and educate the individual on his\/her disability, and the treatment plan established by the case manager, physicians, and therapists.
\u2022 Facilitate treatment between providers and the worker.
\u2022 Provide resources for claims personnel when needing providers in certain locales.
\u2022 Partner with employers determining modified work.
\u2022 Accurately document meeting Best Practices timeframes.
\u2022 Report billing hours in accordance with case activity and billing practices.
\u2022 Follow specific account instructions.
\u2022 Demonstrate professionalism within the work place and in dealing with injured workers.
Qualifications
\u2022 Licenses or Certifications: Current, unencumbered Registered Nurse license required in the resident state; CCM, COHN, CDMS, or CRRN preferred.
\u2022 Minimum of five years clinical experience in a hospital setting.
\u2022 Worker\u2019s compensation or disability management experience is highly desired but not required.
\u2022 Previous field case management experience is preferred but not required.
\u2022 Requires excellent time management and organizational skills.
\u2022 Spanish speaking a plus.
\u2022 Must have reliable transportation.
\u2022 Knowledge of basic computer skills including Excel, Word, and Outlook Email.
\u2022 Must have local unencumbered driver\u2019s license and proof of automobile insurance.
\u2022 Written Abilities: Proficient grammar, sentence structure and written communication skills are required.
Home Office Requirements:
\u2022 Requires DSL, fiber or cable internet connection from home. 1 Mbps preferred or better.
\u2022 Home office must be HIPAA compliant.<\/p>\r\n
